The Sampling and Surveying Handbook is a goldmine of useful tips and procedures for developing and administering your own surveys. As its title states, the handbook also contains good information of the nature of, and procedures for, sampling in support of surveying. The handbook also contains several appendices with adjective groups designed to assist you in developing your own Likert-type survey scales. I edited this book for the US Air Force and contributed to it as well. It is made available free of charge for educational purposes.
